Before you use it
DO NOT GIVE without checking contraindications: myasthenia gravis (can precipitate a myasthenic crisis), decompensated liver disease, previous vestibular or auditory toxicity on an aminoglycoside, and known or family-history mitochondrial mutation (m.1555A>G). Maximum duration is normally 4 days — beyond that, discuss with an infection specialist. Confirm the regimen, the dose interval, the maximum dose cap and the level timing against your trust protocol before prescribing; this calculator gives a dose, not an interval. Ototoxicity is irreversible and is NOT concentration-dependent, so acceptable levels do not mean it is safe to continue. Check levels, check renal function daily, and do not repeat a dose without a level if renal function is changing.
