Warfarin reversal

The two things that reverse warfarin work on completely different clocks. PCC works in minutes and wears off; vitamin K takes 6–12 hours and lasts. Which is why major bleeding gets both, and why giving PCC alone lets the INR climb back overnight.

Before you use it

THIS IS FOR WARFARIN ONLY. A DOAC does not respond to vitamin K and PCC is not its antidote — apixaban and rivaroxaban have andexanet alfa where it is available, dabigatran has idarucizumab, and the INR does not measure any of them, so a "normal" INR excludes nothing. PCC is authorised by haematology in most trusts and the dose band is a starting point, not a prescription. And reversing anticoagulation is only half the job: find and stop the bleeding, and ask why the INR is where it is — missed dose, antibiotic interaction, liver disease, or an intentional overdose.
